For those who will join the face-to-face classes, you are required to submit Pre-admission Medical Examination which includes :
Basic 3 ( Chest X-ray, CBC, and Physical Exam)
NOTE: Those who will will be take Face-to-face classes and those who will participate in the AGWIKASAN are required to take the medical exam. NO drug testing for SHS.
No medical exam for online learners
STEP 1: Download the MEDICAL REFERRAL FORM. You have two options (a) Hi Precision, our partner clinic with a discounted price of 950 pesos inclusive of the basic 5 and Drug Test. For Hi Precision use only, or (b) Referral form for your preferred clinic. Use this this form to any clinic of your choice.

STEP 2: Print and Fill out the form. Bring the referral form to your chosen clinic for the medical exams.
STEP 3: Once result is available. the student must upload it on the link below.
STEP 4: Wait for an email confirming whether your results are CLEARED or Incomplete
